On a cold fall morning in Habersham County, a volunteer found a mother and her teenage son huddled behind a closed storefront — hungry, exhausted, and unsure where they would sleep that night. They weren’t addicts. They weren’t criminals. They were simply a family who had fallen behind, lost their housing, and found themselves with nowhere to go.
This is becoming increasingly common in Habersham County. Every week, Transform Habersham receives calls from individuals and families who are sleeping in cars, tents, abandoned buildings, and on the street — people desperately trying to survive another night with no safe place to exist.
Transform Habersham exists to step in when no one else can or will.

Homelessness in Habersham County has reached a critical point:
Without intervention, many continue to cycle through crisis, poverty, and danger. The community has reached a breaking point — and Transform Habersham is responding to these needs daily.
Transform Habersham serves as the primary point of contact for individuals experiencing homelessness in urgent crisis across Habersham County.
Led by experienced nonprofit founder and community organizer Blake Foster, Transform Habersham brings structure, compassion, and coordination to a fragmented system — ensuring neighbors are not left navigating homelessness alone.

Transform Habersham Inc. is an organization formed by volunteers to help those in our community who are facing destitution and desperation. These people are in dire need of a life transformation because we believe the Bible teaches that they are precious in God’s sight.

- Create permanent, sustainable systems of compassionate support in Habersham County

A Full-Time Community Case Manager is the critical next step.
This professional will:
- Conduct daily outreach to our unsheltered neighbors
- Provide comprehensive, professional case management
- Coordinate mental health and addiction referrals
- Secure emergency shelter placements
- Develop individualized housing and employment plans
- Walk alongside clients until permanent housing and employment is secured
