Walking Beside Our Neighbors in Need

On a cold fall morning in Habersham County, a volunteer found a mother and her teenage son huddled behind a closed storefront — hungry, exhausted, and unsure where they would sleep that night. They weren’t addicts. They weren’t criminals. They were simply a family who had fallen behind, lost their housing, and found themselves with nowhere to go.

This is becoming increasingly common in Habersham County. Every week, Transform Habersham receives calls from individuals and families who are sleeping in cars, tents, abandoned buildings, and on the street — people desperately trying to survive another night with no safe place to exist.

Transform Habersham exists to step in when no one else can or will.

And this is why Transform Habersham needs your help.

Without intervention, many continue to cycle through crisis, poverty, and danger. The community has reached a breaking point — and Transform Habersham is responding to these needs daily.

Transform Habersham serves as the primary point of contact for individuals experiencing homelessness in urgent crisis across Habersham County.

Led by experienced nonprofit founder and community organizer Blake Foster, Transform Habersham brings structure, compassion, and coordination to a fragmented system — ensuring neighbors are not left navigating homelessness alone.

Turning Compassion Into Action

We serve as a bridge — connecting our neighbors experiencing homelessness with the people, places, and resources that can help them rebuild. 

Transform Habersham Inc. is an organization formed by volunteers to help those in our community who are facing destitution and desperation.  These people are in dire need of a life transformation because we believe the Bible teaches that they are precious in God’s sight. 

We provide personal, coordinated support — meeting immediate needs while walking alongside individuals toward long-term stability.

Crisis Intake & Response

We answer calls for help, assess immediate needs, and connect individuals to support through individualized action plans.

Emergency Connections

When possible, we help provide emergency connections and short-term assistance for temporary shelter, food, clothing, job prep, and medical/mental-health services.

Case Management

One-on-one volunteer advocates walk alongside clients through every step as they pursue housing, employment, healthcare, and stability.

Transportation

Transportation is one of the biggest needs for those experiencing homelessness. We provide rides to job interviews, shelters, medical appointments, and rental showings

Community Outreach & Education

We engage local groups, churches, and civic leaders to raise awareness, reduce stigma, and foster compassion throughout Habersham County.

Daily Outreach

We often visit meal programs and known gathering areas to check on clients. Whether it's a talk, prayer, or further support, we are a shoulder they can lean on when they need it most.

Faith & Community Partnerships

We coordinate with churches, nonprofits, agencies, and volunteers who share our heart for serving neighbors in need.

Housing Support

We are actively coordinating with landlords and housing programs for stable housing and assisting clients with applications and move-ins.

Help Hire the Community Case Manager Who Will Change Everything

 Here is where we are today at Transform Habersham. Volunteers are stepping in with compassion. Churches are responding. Community partners are engaged.

But what’s missing is a licensed professional who can bring structure, continuity, and daily accountability to this growing crisis.

A Full-Time Community Case Manager is the critical next step.

This professional will:

  • Conduct daily outreach to our unsheltered neighbors
  • Provide comprehensive, professional case management
  • Coordinate mental health and addiction referrals
  • Secure emergency shelter placements
  • Develop individualized housing and employment plans
  • Walk alongside clients until permanent housing and employment is secured

Without this role, our neighbors cycle through crisis. With this role, lives stabilize.
